A public hearing to support the US Chemical Safety Board’s continuedView of the destruction following the 2005 explosion and fire at the BP Texas City Refinery analysis of effective safety performance indicators and to release preliminary findings into the agency’s investigation of the Macondo well blowout, explosion and fire in the Gulf of Mexico.
