Most Wanted Safety Improvement: Modernize U.S. Process Safety Management Regulations
Process safety management regulations in the U.S. have undergone little reform since their inception in the 1990s. Although recently there have been some positive initital steps taken toward significant improvement in process safety management at the federal level, more must be done to ensure that a more comprehensive process safety management system is in place in the U.S. to protect worker safety, public health, and the environment. The CSB has concluded that implementation of key federal and state CSB safety recommendations will result in significant improvement of Process Safety Management (PSM) regulations in the United States.

As of 2016 the US Chemical Safety Board has issued 12 CSB investigations and 46 resulting recommendations aimed to address deficiencies found in a community’s, facility’s or emergency responder’s response to an incident at a chemical facility.
Most Wanted Safety Improvement: Preventive Maintenance
The CSB has found that inadequate mechanical integrity programs, delayed or deferred preventive maintenance, and the ageing infrastructure of equipment at chemical facilities has been a recurring root cause of incidents investigated by the U.S. Chemical Safety and Hazard investigation Board.
