CSB Releases Final Investigation Report on Three Accidents at the Hoeganaes Iron Powder Facility in Gallatin, Tennessee

The U.S. Chemical Safety Board today released its final report on three accidents that occurred in 2011 at the Hoeganaes powdered metals plant in Gallatin, Tennessee. Flash fires and an explosion killed a total of five workers and injured three others.

U.S. Chemical Safety Board Continues Investigation of Two Accidents at the Hoeganaes Corporation Facility

Nashville, Tennessee, May 11, 2011 – The U.S. Chemical Safety Board (CSB) today released test results confirming preliminary conclusions that two flash fires which occurred at the Hoeganaes Corporation plant in Gallatin, Tennessee—one fatal—involved the combustion of iron powder which had accumulated throughout the facility and became airborne in combustible concentrations. A flash fire on January 31st killed one worker and seriously burned another. A similar fire occurred on March 29th and caused one injury.

Washington DC, February 16, 2011 – A five-person investigation team from the U.S. Chemical Safety Board (CSB) continues its investigation into the scene of a fatal flash fire at Hoeganaes Corporation, a producer of powdered iron located about twenty miles outside of Nashville, TN.
