The final report and recommendations are based on the investigation by the CSB of the April 17, 2013, fire and explosion that killed 15 people and injured 260 others in West, Texas.
January 29, 2016, Washington, DC – Today the U.S. Chemical Safety Board (CSB) released a safety video into the fatal April 17, 2013, fire and explosion at the West Fertilizer Company in West, Texas, which resulted in 15 fatalities, 260 injuries, and widespread community damage.

Waco, Texas January 28, 2016 – Today, the US Chemical Safety Board (CSB) will vote on its final report and safety recommendations on the April 17, 2013, West Fertilizer fire and explosion in West, Texas, which resulted in 15 fatalities, more than 260 injuries, and widespread community damage.
The Chemical Safety Board will hold a public meeting on January 28 in Waco, TX to release its final report into the West Fertilizer fire and explosion. The report is now available for public preview prior to the CSB's meeting.
