Accident Description
One fatality and five injuries resulted from an explosion and fire in a mixing room at the Universal Form Clamp facility. Workers were heating and mixing flammable solvents in an open top tank. During the heating process, there was a sudden evolution of flammable vapors that ignited, resulting in the explosion and fire.
Investigation Status
The CSB's final report was issued at a news conference in Chicago on April 10, 2007.

Amend 1910.106 Flammable and Combustible Liquids to require facilities that handle flammable and combustible liquids to implement the requirements of 1910.38 Emergency Action Plans.

Shortly after 9:00 a.m. on Wednesday, June 14, 2006, an explosion and fire occurred in a mixing room at the facility, causing one fatality and five injuries.

Chicago, Illinois, April 10, 2007 - An explosion and fire took the life of a contract delivery driver and injured two employees at the Universal Form Clamp (UFC) plant in Bellwood, Illinois, on June 14, 2006. The explosion occurred when hazardous vapors, generated by overheating a flammable liquid in an open-top tank, ignited, the U.S. Chemical Safety Board (CSB) found in a Case Study issued today.
